Washington University School of Medicine (WUSM) is committed to providing a safe working environment for all personnel, including individuals who have contact with animals. This includes employees who handle live animals, unpreserved tissues or body fluids, animal cages, or animal carcasses.
Annual screening can help detect those who are developing allergic symptoms so that appropriate intervention can be taken to prevent long-term difficulties.
To participate in the medical surveillance for animal handlers, an employee must complete the fill-able forms below. Then click on “File” to save and send your completed form, and email as a PDF to:OccupationalHealthService@wusm.wustl.edu. All animal handlers must participate in the Medical Surveillance certification, you may enroll in the OHS Medical Surveillance course through Learn@work.
If an employee elects to opt out of completing the annual medical surveillance form there is an option to decline participation.
