Exposure Information for Medical Students/Employees

All animal, human and laboratory blood/body fluid exposures should be reported immediately to the Health Service. The Health Service maintains a 24 hour reporting system.  During working hours, 8am-4pm, the office can be reached at (314) 362-3523 for students and (314) 362-3528 for employees. After hours the Health Service can be contacted through a digital pager at (314) 871-2966.

ANIMAL RELATED EXPOSURES

All animal exposures must be reported to Health Services immediately for treatment.  For exposures occurring Monday through Friday between the hours of 8:00am - 4:00pm, students call 362-3523, employees call 362-3528.  After hours the Health Service can be contacted through a digital pager at 871-2966.      

Student/Employee Responsibilities:

    1. Immediately wash injured area with soap and water. If exposure to mucous membrane, flush the affected area with running water.   
    2. Identify the animal or equipment that caused your injury.    
    3. Inform the area supervisor of the exposure.
    4. Contact Health Services at the number listed above.

HERPES B EXPOSURE 

A student who is exposed by a macaque monkey through a scratch or bite, or was exposed to ANY piece of equipment that may have been exposed to their secretions or blood, requires special attention and adherence to a strict protocol because of the risk of Herpes B infection. 

Student/Employee responsibilities:

    1. Scrub the area immediately with soap and water or betadine for a minimum of 20 MINUTES. The time spent scrubbing or soaking the wound is more important than the type of solution used. Gentle massage of deep wounds can increase contact with the cleaning solution. For an eye splash or mucous membrane exposure,  rinse eyes or mouth well for 20 MINUTES at an eye wash station.
    2. Identify the animal or equipment that caused the injury. 
    3. Report the incident to the supervisor. The supervisor will notify the Veterinarian on call. The Veterinarian will arrange for the monkey blood to be drawn to check for the virus.
    4. Call the Health Service immediately. You will be instructed when to present to Health Service for treatment.

It is very important to report an exposure not only to receive treatment but to know what types of symptoms to observe for. Evidence from previous Herpes B infection suggests that people survive if they are treated early before advanced symptoms develop. 

NEEDLE STICK/HUMAN BLOOD AND BODY FLUID EXPOSURE PROTOCOL

If an exposure occurs, use the following protocol:

    1. Cleanse wound immediately with soap and water. If mucous membrane exposure, rinse with copious amounts of water.
    2. Identify the source of exposure. If the source is known HIV, you must obtain NAME and DOSAGES of medications the source is taking.
    3. Call the Health Services immediately.  Source patient should be evaluated for HIV, HepBSAg, HepCAb. The responsibility of acquiring patient consent for testing will be the responsibility of the Attending Physician.
    4. The student will be instructed when to present to SHS for follow-up. Individuals will be evaluated for HIV/HIV freeze, HepB status, HepCAb, Td status and Post Exposure Prophylaxis. Incident report will be completed at this time.
    5. Exposed student should avoid exchange of body fluids with other persons until follow-up is complete. Condoms should be used as a protective measure with sexual partners until the results of the SOURCE HIV are known.

CHEMICAL EXPOSURES

The following principles should be used for essentially all chemical related exposures:

bulletEye contact: Promptly flush eyes with water for a prolonged period (at least 15 minutes) at eye wash station.
bulletSkin contact: promptly flush the affected area with water (15 minutes) and remove any contaminated clothing.
