HEALTH REQUIREMENTS:

EALTH EXAMINATION FOR NEW STUDENTS
Admission to the University requires a complete medical examination within 1 year of matriculation. If needed, a physical can be obtained at Barnes Care Corporate Health Service. The cost is very reasonable. Please call (314) 747-5800 for an appointment, identify yourself as a Washington University School of Medicine student. An appointment will be scheduled to avoid any delays. This is not affiliated with Washington University School of Medicine Student Health Service.

MMUNITY DOCUMENTATION FOR NEW STUDENTS:
Immunization and test form (please complete these forms in its entirety) All incoming students must provide evidence of the following:
- Tuberculosis Surveillance – Must have a TB skin test within 12 months of matriculation date or visiting rotation. If positive, must supply proof of test and copy of chest x-ray REPORT. A second intradermal tuberculosis skin test will be given at the time of orientation. This test is repeated annually on all students.
- Tetanus-diphtheria OR Tdap – A booster dose is required every ten years
- Measles (Rubeola) antibody - Must supply copy of laboratory report. Vaccination dates are not acceptable
- Rubella antibody - Must supply copy of laboratory report. Vaccination dates are not acceptable
- Mumps antibody - Must supply copy of laboratory report. Vaccination dates are not acceptable
- If negative results, must supply date of MMR or Measles (Rubeola) booster
- Varicella antibody - Must supply copy of laboratory report or vaccination dates. History of illness not acceptable.
If vaccinated must supply proof of vaccination dates for Varivax #1 and Varivax #2.
Required for Medical, Occupation Therapy and Physical Therapy programs
- Hepatitis B Vaccine – Hepatitis B vaccine #1, Hepatitis B vaccine #2, Hepatitis B vaccine #3
- Hepatitis B surface antibody - Must supply copy of laboratory report. Required if completed series and enrolled in Medical, Occupation Therapy or Physical Therapy programs.
Any immunization or test which are incomplete will be given at the student's expense. Noncompliant students will be excluded from classroom and/or patient care areas. |
AST MEDICAL PROBLEMS:
Students having a history of medical problems that may require continued medical attention are urged to have their physician(s) forward relevant information to the address listed below.
A pre-existing condition is any injury, illness, or condition for which you received medical advice, diagnosis, care or treatment during the six (6) months immediately prior to the effective date of your Student Health Services coverage. Student Health will provide coverage for routine, continuing care related to pre-existing conditions (i.e., allergies, dermatology, etc.). There is no Student Health Services coverage for major medical expenses related to a pre-existing condition until you have had Student Health Services coverage for 12 months. There is no pre-existing condition exclusion for pregnancy. |
